The SOURCE Health Equity Initiative

This report outlines The Source System of Care Expansion project‘s health equity initiative. Described are the characteristics of Elkhart County youth and the factors that influence their health. The Source, partners, and agencies throughout Elkhart County, developed an approach to improve behavioral health outcomes in a way that is responsive to the health beliefs and practices, preferred languages, health literacy, and other communication needs.
