Transition Planning and Processes

Planning for the Transition to Work

This page offers information for individuals and their families who are learning more about transition planning. It includes links to resources that will assist you in the planning process.

The transition from high school to work will help you explore the possibilities, but it will also naturally bring up some questions.

  • Who will help me?
  • What will I do with my time?
  • When do I start planning?
  • Where will I work?
  • Why does this feel scary?
  • How can I prepare myself?

While the transition might seem overwhelming, it is also exciting! You are now an adult and can make decisions for yourself. Now let’s reframe those questions above.

  • Who are my biggest supporters?
  • What am I enjoying in my life right now?
  • When do I get to start?
  • Where can I use my strengths?
  • How can I continue to do things I enjoy?

Planning ahead can help ease any fears or uncertainties you may have while preparing for what’s down the road.  When thinking about your next steps to employment, having a trusted team of supporters, or guides, to help you through the process is key. Those team members should have high expectations for your future in mind.

Your team will include those closest to you: you, your family, agency representatives such as employment service providers or a case manager, and anyone else you would like involved in the process. Select team members who have your best interests at heart, who will listen to your dreams for the future, and help you make the best decisions.
